Welcome to Amity Arts!

This form helps us get to know you so we can support you in class, keep everyone safe, and make sure you feel seen and celebrated. You’re welcome to skip anything that doesn’t feel relevant, and if you’re not sure how to answer something, that’s totally okay. We’re here to listen, not judge—and we’ll always treat your information with care, respect, and confidentiality.

Student Participation Agreement and Declaration

Participation is subject to review, and we reserve the right to refuse, withdraw, or adjust access to classes based on the information disclosed, or in response to behaviours or incidents that arise during sessions, in line with our duty of care.

Amity Arts follows UK safeguarding law. Any concerns about abuse or neglect will be reported in line with our Safeguarding Policy.

Payment for your place within classes is £99.64 monthly made via direct debit, and by submitting this form you agree to sign up to and maintain a valid direct debit arrangement for the duration of your participation. Class fees are reviewed periodically and may be subject to change with notice, in line with our commitment to sustainability and accessibility.

Amity Arts is a drama education provider. We do not offer medical care, personal care, or community support services. By submitting this form, you acknowledge that any support needs beyond education in the classroom—such as health, personal care, or social services—remain the responsibility of external providers, families, or support workers.
